University Life Programming Grant Application Form

Requests for University Life Programming Grants must provide information on the following. For additional information, please see our program proposal information.

Information may be submitted online by completing the form below, or you may print the form and mail it, fax it, or save it as a Word document and email it to the address below. If you complete the form offline, please limit submissions to no more than 2 pages.
